Application Forms:

These are the forms you will require for your Hellerwork Selection Committee.

Practitioner Agreement:

Please read this agreement including Attachments A, B, C, D listed at the end of the agreement. Print two copies and bring to your selection. This will be your contract with Hellerwork International. One copy for you and one to be sent to Hellerwork International and will come into force upon graduation.

Training Application:

Please download and fill out. Email it back to info@northwesthellerwork.com at least 3 days before your selection committee date. If you chose to mail it in hard copy, it may be mailed to: Dr. Don St. John, 3210 NE 167th Street, Lake Forest Park, Washington, 98155, USA. (206) 368-8145.
